This Regulation of the Minister of Agriculture and Rural Development, which is composed of three paragraphs and four Annexes, establishes: (1) forms of certificates of the evaluation of seed material and form of certificate of the seed mixture (specified in Annex 1); (2) forms of information on disqualification of seed material in individual phases of evaluation (specified in Annex 2); form of the extract from certificates of field evaluation of seed material (specified in Annex 3); form of information on results of examination (specified in Annex 4).
